Clinical needs come first
The patient’s condition determines the medical team, equipment and suitable method of travel.
A medical transfer can be an uncertain time for a family. One hospital may be ready to discharge or transfer a patient, while the receiving team, transport and travel arrangements are still being confirmed.
SAM was established to manage that gap carefully. Our clinicians review the patient’s current support needs. Our coordinators arrange the transport, access, documents and timings. Together, they help maintain continuity from the sending bedside to the receiving team.
We support families directly and work with hospitals, insurers, assistance companies and transport providers. Each transfer is considered on its own clinical and practical circumstances.
How we work
The patient’s condition determines the medical team, equipment and suitable method of travel.
Access, timing, documents and receiving arrangements are checked before departure wherever circumstances allow.
One coordination team provides practical updates and explains the decisions that affect the transfer.
The journey includes a structured clinical handover, not simply arrival at a destination.
